Due to the exclusionary policies of most major medical insurance plans for new physical therapy providers for the 34711 zip code area, we are unable to participate as in-network providers, EXCEPT for the following plans:

United Healthcare

Department of Labor OWCP DFEC Workers' Compensation
We may be able to participate as out-of-network physical therapy providers if available within your plan.
If not, we do offer pre-paid specials when self-paying for multiple sessions at once.

Insurance coverage is for PHYSICAL THERAPY services only and does not cover our Personal Training and Body Tempering services.
